With his number one option Matthew Kreuzer sidelined with a thumb injury, Malthouse says he is considering bringing both experienced Robbie Warnock and youngster Levi Casboult into the team for the match against the Eagles.
Along with forwards Shaun Hampson and Sam Rowe, both of whom can switch into the ruck if required, it would mean several Blues taking turns squaring up to the competition's in-form big man.
Cox was outstanding in the Eagles' 94-point win over Melbourne last weekend, especially in the second half as he dominated Demons' ruckman Mark Jamar and gave his team continual first use of the ball.
"We understand we're a little bit short in terms of talls in our side ... we might go a little bit taller," Malthouse said on Wednesday, saying he could decide to play both Warnock and Casboult.
"You're talking about the best ruckman in the league - Cox has been that for a number of years.
"We know we're going to have a big job, and perhaps the best way to do that is multi-choice.
"It's very hard to lock one bloke in on a player like Cox because he's so experienced and he's so good."
With most pundits believing his winless team will return from Perth at 0-4 for the season, Malthouse is refusing to panic.
He says once his side gets stronger for longer, the wins will come for players still adapting to their new coach's demands.
"It's not about win-loss, it's about process," Malthouse said.
"Are we playing good football? Yes, we are in patches.
"Are we playing well enough to beat our opponents? No, because we're not doing that for a full game.
"That's what we have to look at, not (just) the result of the football game."
Malthouse has also ruled out an immediate upgrade for any player from the club's rookie list, with Nick Graham's knee injury creating a vacancy should the Blues decide to elevate someone.
