Tuohy, 21, hails from County Laois near Dublin and spent last year learning the game after being recruited as an international selection with the 73rd overall pick of the 2010 rookie draft.
He was elevated to the senior list earlier this season and travelled to Sydney for the round six clash against the Swans as an emergency. However, he was returned to the rookie list when first-year forward Luke Mitchell recovered from shoulder surgery and was taken off the long-term injury list in May.
In making his debut, Tuohy will become the 20th Irishman (including Sean Wight and Setanta O'hAilpin*) in VFL/AFL history to play at the elite level.
The former Gaelic footballer has been named in the Northern Bullants' best players in four of the seven VFL matches he's played this season, to earn a second elevation and his AFL debut.
"I am very excited and really looking forward to the game as it is a big step in my career," Tuohy told carltonfc.com.au on Wednesday.
"Having my parents our here in Australia adds to the occasion for me and it will be great to have them in the stands in Adelaide."
Tuohy, who is 187cm tall, has played predominantly on the ball and across half-back in the VFL and Carlton football manager Steven Icke said the club had been delighted with his progress since he joined the club.
"His improvement over the past two years has been rapid," Icke said.
"He has genuine speed, is an elite kick and his selection this week is well-deserved."
Tuohy's elevation comes at the expense of Mitchell once again, with the former Calder Cannon requiring further surgery on the same shoulder, which has seen him returned to the long-term injury list.
