Carlton will receive Adelaide's round-two pick (no.33 overall) and a round-four pick (no.67 overall).
Jacobs requested the trade back to his home state before trade week began, and the Crows said they were keen to strike a deal rather than force Jacobs, who has played just 17 games, into December's pre-season draft.
"We're very happy for Sam as he was desperately wanting to get home to Adelaide and he felt that because Carlton have four big ruckmen his opportunities there were limited," Crows football operations manager Phil Harper told afl.com.au's Trade Week Radio on Monday morning.
"To be able to come up with a deal that satisfies Carlton is terrific.
"It made them happy, it makes Sam happy and of course we're happy as well, which is great."
Jacobs is another to progress from the rookie list, after Carlton took him with the first pick in the 2007 rookie draft.
He played two years at Carlton's AFL affiliate club the Northern Bullants, before being delisted and redrafted with pick 72 in the 2009 rookie draft.
Jacobs was elevated to the senior list for the 2010 season, playing four senior games in 2009 and 13 this year.
