Shaw's set shot during the second quarter against the Brisbane Lions clearly hit the post but was awarded a goal, even after a consultation between the goal, field and two boundary umpires.
AFL umpires manager Jeff Gieschen said several mistakes were made in the lead-up to the goal being signalled, starting with the positioning of the goal umpire.
"The umpire wasn't in best position," Gieschen said on his weekly afl.com.au show OPSM What's Your Decision?
"[He] was looking from side-on.
"He should have stayed behind the flight of the ball, which was near the post, and he would have been able to see that little bit of a flick."
Gieschen said even with the initial mistake, he had hoped one of the other six umpires within clear viewing distance could have stepped in.
"If you look closely at the vision, there was a boundary umpire on each post, we had the goal umpire in the middle of the goals, the field umpire about 20 metres out from goal, we had two boundary umpires out on the 50 metre arcs, and we had another field umpire in the middle of the ground," Gieschen said.
"Between those six or seven umpires, we would have expected that at least one of them would have had a little bit of a feel that the ball had hit the post, so that's the disappointing part."
The umpires boss also revealed that under guidelines given to umpires, the kick should have been called a behind even if there was doubt about whether it had hit the post.
"Normally when we consult and there's doubt, we go to the lesser score, which had we done in that case, we would have said, 'all clear, one point'," Gieschen said.
"By the field umpire and goal umpire coming together, there must have been some doubt.
"So we're disappointed. We would have liked it to have been handled a bit differently."
Gieschen said the goal umpire would now be sent back to state level to "work on his form and confidence".
"We have very high standards," he said.
"So when we see ones that are pretty basic like that one, there's an expectation that goal umpires at this level will get that."
