Jacobs, 22, is one of four big men on the Blues’ senior list and has been the subject of trade speculation since Carlton's season ended.
The South Australian, who was among his side’s best in its elimination final loss to the Sydney Swans, finished the season out of contract and his good form prompted the interest of several clubs.
He and teammate Andrew Walker - who has also been linked to a move to Neil Craig’s side - travelled to Adelaide earlier this month to meet with Crow officials.
Carlton had hoped to convince Jacobs to stay at Visy Park and continue working with fellow big men Matthew Kreuzer, Robbie Warnock and Shaun Hampson.
However the improving ruckman informed Blues officials of his desire to return to his home state - and his preference to play for the Crows.
“While disappointed with Sam’s decision to return back to South Australia, the club will now commence discussions with the Adelaide Crows to enable a suitable exchange,” Carlton football manager Steven Icke said.
While the AFL exchange period doesn’t officially kick off until October 5, Blues officials can now begin brokering a deal with Crows officials.
The two parties will be in Canberra this week for the NAB AFL Draft Combine, where the best young talent in Australia will gather to be tested in front of club officials and recruiters.
Jacobs played 17 AFL matches for Carlton since making his debut in round one, 2009.
He was recruited to the club from Woodville-West Torrens in the 2007 rookie draft.
